Client Rights

  • Informed Consent: You have the right to be fully informed about your treatment options, including the benefits, risks, and side effects of any therapies, and to make informed decisions about your care.

  • Right to Privacy and Confidentiality: You have the right to expect that your personal information and treatment records will be kept confidential and will not be released without your specific, written consent, except in specific circumstances outlined by the law. You have the right to be informed of any photograph, audio, or video recording made of you. You have the right to refuse any recording except for identification or supervision purposes.

  • Right to Information: You have the right to receive clear, concise, and understandable information about your diagnosis, treatment options, potential benefits and risks, and costs.

  • Right to Participate in Treatment Planning: You have the right to be involved in developing your treatment plan and to express your preferences and concerns.

  • Participation in Treatment Decisions: You have the right to be actively involved in developing and reviewing your treatment plan(s) and to refuse treatment, except when a law dictates otherwise.

  • Least Restrictive Environment: You have the right to receive services in the setting that is least restrictive to your freedom.

  • Right to Fair Treatment: You have the right to be treated fairly, with dignity, respect, and courtesy in all interactions with staff and other clients.

  • Access to Records: You have the right to access your treatment records. You will be instructed on how to make this request. 

  • Right to Refuse Treatment: You have the right to refuse treatment, including medication or therapy, unless you are involuntarily committed or there are legally defined exceptions.


Client Responsibilities

  • Active Participation: You are responsible for actively participating in your treatment, including providing accurate information to your therapist, attending appointments, and following treatment plans.

  • Respect for Others: You are expected to respect the rights, privacy, and dignity of other clients and staff members.

  • Following Treatment Plans: You are expected to follow the agreed-upon treatment plan(s) and to seek clarification if you have any questions or concerns.

  • Financial Obligations: You are responsible for fulfilling your financial obligations related to treatment.

  • Keeping Appointments: You are responsible for keeping appointments and providing adequate notice if you need to reschedule or cancel.

  • Transparency and Honesty: You are expected to be transparent and honest about your condition and any changes that might affect your treatment. 

  • Confidentiality: You are expected to maintain the confidentiality of other clients and participants in a group setting.
